Generic application - Family composition

I had submitted mine and dependents medicals during July 2012 to NDVO. I informed them about Family composition and date of delivery is 22 March 2013. CHC sent me a letter by asking me to fill the below forms:

- an IMM8 application form and Family Information Form filled in for the child
- updated IMM0008 Generic and updated IMM5406 “Additional Family Information” form for yourself and your spouse

Both the lines above are pointing same forms or different? IMM0008 Generic can be filled again for each dependents???

Also they mentioned medicals are valid up to 23 July 2013 and there will be no extention given whenever the visa is issued? Will it mean if any delay occurs they will not exention the visa or is there any way to extend the visa? Anyhow I am trying submit the documents as soon as possible after the family compoisiton.

I have to wait for the medical request for the new born after submitting the above doucments. Our medical were done paper based, do I have to get online medical for the new born or paper based like us.

Please clarify.

I have been through the same situation.

You just need to submit


updated IMM08, IMM5406 forms
passport and birth certificate copies for your new child
$150 processing fee
passport-sized photos of your new child

You will get MR for ur little angel through email within a month or less after submitting these docs. In Sha Allah!

Jazack Allah Khairan for the swift response.

IMM0008 Generic should be filled for principle applicant and all the depenedents including new born. Am I right, if not correct me.

The required forms imm0008egen.pdf, imm0008_1e.pdf and IMM5406E.PDF

I didn't find the form imm0008_1e.pdf.

CHC asks for a notarized birth certificate. I am in Saudi Arabia, how can I get notarized certificate, here all the advocates or lawyer are Saudis.

Will they issue visa soon after submitting all the passports as the deadline is 23 July 2013. If any delay occurs either CHC side or my side is there any possibilities to extend the visa?
