GCMS Notes

Hi there,
I am Canadian citizen and sponsoring my wife from Pakistan and I want to order GCMS notes for our application status and my question is that can I email consent form to my wife and when she sign it does she have to mail it back to me or will they accept if she sign it then scan it and email it to me.

Hi there,

Tell her to print, sign and scan it to you. It will be ok by experience. I did the same and receive my GCMS note.

i am going to apply for gcms notes and looking for help to fill out the forms.thx in advance
below are problems....

under qestion number 3.
if you already submitted your application:
- name of office where the application was submittted= ? (i filled new delhi, not sure wrong or right, plz suggest)
- location of office=?
- under declaration, should my wife sign as well as she is going to apply for gcms or just my signature are needed?

- should i click on access to information act or privacy act?

yes or no? (my wife is applying for me. i am in india and she is canadian citizen)
2 select the category that best descibe you.
i clicked public (is it ok?)
3. business or organization name(if applicable) i left blank.(is it ok?)

under last column signature and date.
should my wife sign this ? coz she is applying from canada.

I doubt you are using old forms. Now you need to submit request online and need to fill up one form that is IMM 5744. just visit the link >>> https://atip-aiprp.apps.gc.ca/atip/welcome.do
