Hi All,

I want to order GCMS notes.

I have applied with non- accompanying spouse.So my spouse information is there in the application. But, I will be only getting PPR/PR.

So, what should I put in section "2.1 Related Individual's information" in consent form?

Should it be blank or I need to put details of my spouse?

For you to be able to see her info, she must sign the consent form with you.

If the child is below 16, both parents must sign. If 16 and above, the child signs.
