GCMS Notes error after requesting

Hey Everyone,

So I've requested the GCMS notes and I got an email from IRCC (Formerly known as CIC). Here's what they said.
We regret to inform you that we are unable to process your request for the following reason:

· The consent provided was modifiable. Please provide an authentically signed non-modifiable consent from (Spouse's Name). To be valid, a consent must be signed, recently dated, contain the printed name of the person giving consent and identifies you as the designated individual.


Basically this was the process. I emailed her the form. She filled it out then printed it. She signed it and scanned it and emailed it back to me.

I printed it out and signed it too then scanned it and uploaded it to the request form. Is it because only our signatures are done by hand and the form was filled in electronically?

To be honest I have no idea where the process went wrong. It seems that everything has been done properly.

The consent form is supposed to be signed by the principal applicant alone. Your application was rejected because it had two different signatures.

Thanks for the prompt reply!

Would it matter if I told her to download the form again, fill electronically and sign at the bottom and scan it and send to me would this work?

The "modifiable" is because you printed it out, signed on top of it, and then scanned again a second time. This would be very obvious, and from a due diligence perspective, CIC deemed that to be a problem...someone could modify the original.

Now, other posters have said that only your wife has to sign it and not you. I have also seen other posters saying that you also have to sign in the spouse section as there would be information pertaining to the spouse in the notes, hence the need for the spouse to also sign to release that.

I really don't know. Since the application is already done, only waiting for the signed form, and other folks have said only your wife needs to sign. What you can do is as your wife to fill up and sign, scan and mail to CIC. You can forward her the rejection email, and then she forwards over the scanned attachment herself.

First time I ordered notes, I signed also in the spouse section. 2nd time I ordered notes, only the applicant section was signed...that said, by that time, I was already the representative for my spouse with the additional form signed allowing me to access personal information. So it may not even need her signature...but I wanted to be safe and get her signature as the notes take 30 days and I didn't want a delay.

Makes sense,

What I did in this case was give her a new copy of the form and she printed it out and filled it in by hand entirely.

She signed for both sections and I have replied to IRCC. Hopefully it all goes through well ;D
