GCMS NOTES Consent Form Issue

Hello

I applied for GCMS notes for me (PA) and my wife (Dependent). I filled the consent form and submitted. But today i recieved the email back with following issue:

I regret to inform you that we are unable to process your request for the following reason:

A new consent from <my wife's name> is required to process your request. To be valid, a consent must be signed, recently dated, and indicate to whom authority is given with the printed name of the person giving consent. Please, complete the section C of the consent.

What i filled in consent form:
Section A: PA information
Section B: Wife's Information
Section C: 3. Blank 4. checked "Email" option
Section D: Blank

I want to give them PA information and my wife's information as well.

Please let me know how to fill the consent form. Thanks

1. As the CIC email states, you would need also to complete Section C - Part 3 also.

2. It seems you are intending to get the Info to yourself, so the Best way would be to complete a new form with Part-1 filled for your wife, Part - 2 : Blank, Part-3 filled with your information, Part-4 checked for email. The form is complete with your wife signature.

3. BTW when did you send your request for this GCMS notes and when did CIC come back to you?? Just for info. Thanks for sharing.

i have applied and it worked for me
Section A - ur wife information (spouse)
Section B - Blank
Section C - ur information(PA)
Section D - Email
Section E - Blank
Section F - ue wife signature
Done scan it and upload it , all the best

What consent form is this? Doesn't the main application forms already include dependents' information and each dependent have to sign? Is this a new consent form that CIC just came up with??
