Actually I have an agent kaso they are not recognized by CIC. As per CIC any "paid" agent should be in good standing with them..
meaning an Agent should have membership ID or have a CSIS # in order for agent to represent you.

So sa madaling salita, ka cancel ko na kasi my agent is not able/he can not represent me (legally with CIC). And CIC is requiring me this form IMM5476.

So quite confusing dun sa part na yun..

Name of office where the application was submitted - if Embassy of Canada or the name of Agent?

hi batchmates... i called Timbol Clinic in Makati for our medical.. referred yun by a friend who migrated na din sa Canada. Ang kailangan dalhin ay vaccination record if you have kids and biopsy or histopath reports if you have previous operations within the last 5 years. We are asked to bring also our reading glasses.. We will have our medical this wednesday. If ever dun kayo sabay na tayo ha..

this person did all mental harrasement to me and my family. he has used all bad words which a genune person cant even think to use to a lady. he got very frastated so please ignore him for any future communication. he can even hack ur id and can do any thing with ur system IP address. he spoiled my whole sunday and I am kindly requesting you all not to follow him at any stage of time

San ka dito sa atin mag pa med exam? Mauna ka na muna kasi hindi pa ako makahanap ng time, duty kasi..and dapat mag laan talaga kami ng whole day kasi family of six kami.. Cge, balitaan mo ako ha , kung ano ano yung mga ni-require sa labs..and kung magkano din yung mabayaran..Plan namin sa Nationwide ky Dr. Baruiz..

Nakapag-inquire na ako last week P4,415 per head ang cost kasali na daw dun ang pagpapamail sa result. Bali eto yung kailangang test na i-undergo: Xray,Urine test,HIV test (for 15 yrs old and above) and PE.

hi,pwede naman po kayo kumuha ng bank draft o"managers check" basta po meron kayong savings account po.basta po dun kayo sa branch nyo kung saan kayo meron kayong account.

Nakapag-inquire na ako last week P4,415 per head ang cost kasali na daw dun ang pagpapamail sa result. hi just for the info st.lukes fee is 4,200 per head sila na din po bahala magsend sa CEM same procedure din po.

In the the form IMM5476 - The Use of Representative form

Ano po pagkaintindi nyo (Name of office where the application was submitted?)
Is this referring to the Agency/Immigration consultant you hired? or the Embassy of Canada, Manila?

i agree with jho. just put Not appicable or if you have an eternal immigration firm, their name should be placed there but take note it should be a legit one acknowledged by the CIC

i c, pero i was just wondering bakit pinapasubmit ka nito ulit, baka alam ng CEM na plan mo na talaga i-cancel yung services ng agent mo or nagkataon lang.. in which case, apart from Section A, dun ka naman sa section C mag update.
sa pagkakaintindi ko, kaya cguro inilagay ang 'Not Applicable' dyan sa part na yan sa form ko dati kasi initial application lang yung ipinadala sa CIC-Nova Scotia so wala pa silang record sa akin.

sa case mo, parang follow-up request nila yan sa 'yo, so lagyan mo nalang ng CIC (as suggested by Mike) kasi sa kanila naman eventually napupunta lahat ng application..yung agent ay 'middleman' lang so yung pangalan ng agent hanggang dun lang sa Section B mo pwede ilagay.
