Hello luckycharm!
1. 4,000PHP for adults. Sa kids yata 3000PHP. Kase wala naman HIV test ang mga bata. 1000PHP kase para sa HIV test.
2. In order ng ginawa sa akin - height, weight and eyesight, blood and urine test, interview ng doctor then physical examination, lastly x-ray
3. Letter from the embassy na for medical na kayo, 4 passport size pictures(white or blue backround only) at photocopy ng passport.
4. I don't have kids yet pero ang alam ko dapat mong dalhing ang immunization records ng mga bata.
Additional tips:
1. Be early. Medyo madaming form ang fifill-upan. Firsr come, first served sila. Hindi na kailangan tumawag sa nationwide for appointment.
2. No need for fasting. Pwede kumain.
3. Drink a lot of water para sigurado na maging clear ang urine. Magbaon kayo ng tubig. Inom lang ng inom habang nasa Nationwide kayo. hehe!
4. For females, dapat 7 days after ng last day ng period pwede magpa-medical.
5. Additional 50PHP per person for courier fee kung sa CEM ipapadala. Kung sa ibang Visa Office, iba ang presyo kaya inform nationwide immediately.
6. Eyeglasses or contact lenses kung meroon.
7. Previous medical reports kung meroon.
