Actually I was given a 2 year diploma by WES. Then ang nakalagay dun na major is nursing assistant. Yun din ang isang reason why I chose to apply as LPN (3233) instead of RN (3012) Kahit ang title ko tlaga sa work is staff nurse.

I agree with this. More likely talaga, your JD as a staff nurse fits also the JD of LPNs.

"Additional Dependants / Declaration (IMM 0008DEP) (if applicable)"

i-eenclude ko ba pa yung form?
i-e-x ko ba yung box or leave it blank?

Pakisagot naman po.

If no dependents, then this form is not required. So I excluded this form.

If you're JD fits NOC 2011's description for the duties and responsibilities under 3012 then i guess you won't have a problem on that aspect. I have heard a lot, however, that the reason for rejection might be due to degree equivalency. Some Filipino nurses were evaluated to have degrees equivalent to what LPNs in canada have. Well, im not sure about this thing. I applied for 3012 as well, but i have a masters degree. I was given by WES an evaluation of 4 year Bachelor's degree. My work is a nurse researcher and basically my JD describes what a nurse researcher does. If you look at it at the NOC 2011 description, a nurse researcher was described very briefly, one line nga lang yata. But I got a positive assessment from CIO and now i'm waiting for my MR. Just to be safe, check your degree equivalency also aside from the JD you have.

NURSES, TAKE NOTE:

The NOC code employment requirements of HRSDC does not have bearing in the FSW process. What is stated in the HRSDC website are the Canadian standards and this does not apply to other countries. This has been proven and discussed several times in the international threads. What is important is the job description. Even the job title is not that important. IOs will use the JD found at the HRSDC website to match your description to what you claim your NOC code is. This style has been successfully done by several people in the IT NOCs wherein there are overlapping JDs (like 2174, 2173, etc)

A good example is roman 4:21's case; A 3011 (nurse co-ordinators and supervisors) applicant who has a WES degree equivalent of 2 years diploma. He has just received MR last week.

Do you mind sharing your JD so that the 3012 aspirants here would have a benchmark on that crucial document for claiming 3233?

I patterned it under the JD for NOC 3233 in the website for NOCs

What do you mean by this?
According to them their app was returned because of the cap.

189 palang ang 3012 CAP as of last week. If you do monitor the international threads, it is obvious that CIC follows a pattern. Madaling paniwalaan Sa 2174 Kasi naging evident sila lalo na sa SS pero ang 3012 na Cap reached ngayon, that's something...

I hope you could get specific details in their application like received date, etc. This could be very helpful to the community.
