you have to get the latest one. You can authorized someone to get it for you. Some banks have their forms to fill out in order to process your request for bank statement as well as bank certificate. I had sent authorization letter in Philippines to get it for me. They gave them the bank certificate and bank statement. I just have to give them the forms along with my signature in the said forms.mpmalvar said:

yes po.it must be original. Like I said ,the forms are specific per bank. Ask your bank first, because last december when I went for my yearly vacation, I got the form from the bank and signed it and filled out the required data/information.mpmalvar said:
