1) Yes
2) I prepared the BSF186/A forms (the main bsf186 page and 1 additional page of bsf186a for items brought with me; and the rest of the additional bsf186a pages for items to follow). I then photocopied the whole stack. They just asked me to sum up the total value of everything at customs - you can do it at home to fasten the process.
*Make sure you tell the customs checkpoint that you have goods to declare and that you want to get those forms stamped. They just wave people by without properly checking.
3) Just passport and COPR. They didn't ask for proof of funds but just asked how much cash I was carrying - I had 20 bucks
. If they ask, you should be able to present a recent bank statement or present your account from a mobile app or something.
