FSW - What would be "To"date for my current residence

I am little confused while filling up the forms. In case of my current job or residence what would to "To" date. I will keep it empty or write "date" or have to put date of signature.

I would appreciate if someone clarify me.

Should be the current date....i.e. MAY 2013
