guys, just want to clarify lang sa MEDICAL REPORT: Section A form,

with our picture & datas, may portion sa ilalim na nakalagay CONTACT ADDRESS/ PERSON WITHIN CANADA, kasi blank lang yung sa akin,

do i have to fill that up? what does that mean?

Honestly, I was not able to ask them but in DMP's handbook it says that blood test is for syphilis and HIV for 15yrs old above.But in my case I had to undergone additional creatinine test since I declared my hypertension. ECG and creatinine test are mandatory for hypertension and other diseases declared, or in case you're blood pressure is elavated during physical exam. Here's our experience sa Nationwide:
1. First get Forms sa reception desk pa lang, show medical request and passports.
2. Fill-up Forms, medyo marami, like what other says bring black ballpens, don't forget to paste pictures may glue na mahihiram naman dun.
3. Bring Forms to table 2 for assessment then wait to be called for ID number and checklist.Dito medyo matagal lalo na napansin ko yung iba dami kulang sa forms nila.
4. Pay to the cashier
5. Immediately after payment we're called for height, weight, eye exam (oops ;D di ko mabasa need ko na eyeglasses)if you're wearing eyeglasses make sure to bring them.
6. Then, doctor's physical exam, I was suprised mabilis lang, got some basic questions based on the form,she asked my previous medical record since I undergone cholecystectomy but did not ask for cesarian record which 13yrs ago pa. BP and then undress except undies for breast exam and pisil pisil lang tummy at puson, before leaving she explained I had to have additional ECG and creatinine exam since I declared na hypertensive ako kahit yung BP ko ay 130/90 lang. Yung son ko na 12yrs old di naman tinanong immunization record, but for physical exam hubad lahat pati sa husband ko, I guess pag men ganun talaga :D
7. Next was the urine and blood extraction and ECG in my case.
8. Then last yung X-ray medyo matagal maghintay dito, naglunch muna yata yung technician hehe :D
9. Pag tapos na lahat make sure yung checklist nyo may sign lahat proof na tapos na, before leaving give checklist and ID sa receptionist.

In general ok naman medical, di naman nakakatakot hehehe ;D, I just hope na maging ok lahat results and no more further tests for us. Maybe it's not wise to go on mondays kasi nga maraming tao. Just don't forget to bring all that you need like medical request and forms from CIC, pics, drinking water,passport and photocopy of it, medical records if applicable. So to everybody goodluck and stay healthy! God is good!

Hello envogue, yung sa amin wala rin nakalagay pero wala na kami sinulat dun.
 
hi, thanks for the info. sa april 11 kami magmedical sa nationwide. ganun din po case ng husband ko. ideclare nya na may hypertension at diabetes sya so definite na mag ecg at creatinine sya. how much po binayaran nyo for the ecg at crea. P4,050 lang kasi pag walang additional test. Sa nationwide clinic din ba ginawa ang ecg or pinapunta pa kayo sa ibang hospital?

If I'm not mistaken, nursemich advised to register early 'coz there are plenty of steps we should take if we want our credentials recognized in Canada and CIIP will guide us.
 
i think the seminar would also help us prepare for an interview (in case hindi waive :-[) if we attend at an earlier date.

Hi, wala rin nakasulat dun and we're not filling it up. I think hindi natin dapat sulatan yun kasi for the DMP yung Section A. :D
