Please read the guide as posted by westpoint above several times, this is very important, I'm also with an agency and I have filled up one for me and 1 for my husband, and may nabasa din ako di lang ako sure kung saang forum yun na binalik docs nya since hindi rin nag fillup yung husband nya, they only submitted 1 form for the principal applicant, so binalik sicne kulang and di rin kasi nya alam na dapat pala pati spouse, minsan kasi nakakaconfuse yung instructions nila, kaya if may 2nd thoughts kaya, open nyo agad sa forum, para walang masayang na time

Lizz, ganyan din feelings ko nung nakakabasa ako ng mga recent status ng mga nauna sakin (sa ibang forum na di ako member). sabi nga ng lolo ko dati, ang buhay daw para daw Tour of Luzon (Marlboro Tour)... unahan lang sa finish line at ung pagkuha ng yellow jersey. hahaha! pero para dikayo mainip, keep urself busy nalang sa ibang bagay like magworkout/exercise, magtanim ng plant, magfishing, etc. in that way di nio mamamalayan parating na ung mga PERs/MRs nio. pray hard!

From my concern of a Medical Request not being sent, or, post office is way tooooo slow, I sent an email last Monday to Manila VO asking if a Medical Request has been sent.

Alas, I got a reply. and they mentioned, a medical request has been sent and request for right of permanent residence fee last Feb 7, 2011!!!

My next step, I will go to our post office and ask for it.

when u read the however part u will prepare separte forms but when u base it on her experience,she only submitted 1 copy (and that being the applicant) and then it went through though.

to share mine, my agency made us filled up separte forms, in my case only me and my spouse filled up coz our children are below 18y of age.

hello, hindi ba maconfused ang CIO kung makita nila tatlo or dalawang iba't ibang pangalan na applicant sa Additional Family Information form? baka itanong nila who is really the applicant? but personally noh I would stick to their guidelines nga dapat each family member has to fill up the form.

I was just wondering paano ginawa ni ladyk at nakapasa cya na isa lang form sinumbit nya at ibang applicant binalik ang kanilang applications because kulang kasi hindi nagfill up yung spouse nila.

confusing talaga ang guidelines nila.

the way yung pagreply ni ladyk seems na hindi sya sure di ba, kasi baka naman nakalimutan nya na details nya on filling up the form since ilang months na yun

may nabasa din ako di lang ako sure kung saan yun (eto naman same dialogue ko) marami kasi me sources kaya kung saan-saan na ako nakahagilap ng mga info, may nagtanong din same ng tanong mo na ang ilalagay sa form to be filled up ng spouse is magiging sya ang PA and ang PA naman maging spouse, ganun talaga daw yun

sana pwede nalang i pick-up ang MR's natin sa CEM no? di ba natin pwedeng itanong kung pwede yun?di kaya ipa courier tas tau nalang pay upon delivery?hassle kase ang post office eh..

My conclusion is, it is not wrong to overdo it. Like with this Additional Family Information and writing all the 3 NOC's in the mailing label, in my opinion, it is better to make a copy for the principal applicant and each member of the family and write 3 NOC's in the mailing label.

See, I told you!!!!!!! ;D Yours was issued 4 days earlier than mine and I held it in my hands and then to my chest (oh gosh!) on the 18th.
