FSW 2014: Complete set of forms and documents required.......

Dear Friends,
I am going to apply on Monday with the following documents. Please tell me if they are correct else I may have missed something…. Eagerly waiting for your most valuable suggestion……. Please suggest if anything need to be included.
1. Cover letter explaining few points like I am citizen of Nepal and live in India as per treaty between Nepal and India, where no travel document required for the same and henceforth I have mentioned my status for current country of residence as “Free migrant”. Also a few other points if applicable.

3. Barcode page of Generic Application Form for Canada (IMM 0008): (Principal Applicant)
4. Schedule A: Background / Declaration (IMM 5669): (Principal Applicant): Here my name will be in applicant field and my wife’s name will be in spouse field. Correct me if I am wrong.
5. Schedule A: Background / Declaration (IMM 5669): (Spouse): Here my wife’s name will be in applicant field and my name will be in spouse field. Correct me if I am wrong.
6. Additional Dependants/Declaration (IMM 0008DEP): Not Applicable: Only one child.
7. Supplementary Information – Your travels (IMM 5562): Travel details for me, my wife and my child even though we travelled between India and Nepal. I will try to fill 1 travel per year which is approximately accurate.
8. Schedule 3: Economic Classes – Federal Skilled Worker (IMM 0008 – Schedule 3): (Principal Applicant)
9. Additional Family Information (IMM 5406): (Principal Applicant)
10. Additional Family Information (IMM 5406): (Spouse)
11. Use of a Representative (IMM 5476): Not Applicable
Other Documents:
12. Photocopies of Passport (Applicant, Spouse and Dependent): This will be plain photocopy, neither certified nor notarized.
13. Original IELTS TRF.
14. Letter of attestation: Not applicable
15. Arranged employment: Not applicable
16. Work Experience:
a. 1st Company: Offer letter, Confirmation letter, Relieving letter, pay slips of 3 months and Job description which has all duties (I had signed this job description while taking the post). All in original letter head. However the company does not provide the reference letter as specified in document checklist. (All photocopies only and not notarized)
b. 2nd Company: Offer letter, Confirmation letter, Relieving letter, pay slips of 3 months and reference letter as specified in document checklist. They agreed to give me the reference letter and sent me the soft copy which I will take printout and include in the package. (All photocopies only and not notarized)
c. 3rd Company: Offer letter, Confirmation letter, Relieving letter, pay slips of 3 months and Job description which has all duties (I had signed this job description while taking the post). All in original letter head. However the company does not provide the reference letter as specified in document checklist. I have written mail communication from HR manager that they do not provide such kind of document. The company is almost largest IT Company of the world. (All photocopies only and not notarized)
d. 4th Company: Offer letter, Confirmation letter, Relieving letter, pay slips of 3 months and Job description which has all duties (I had signed this job description while taking the post). All in original letter head. However the company does not provide the reference letter as specified in document checklist. (All photocopies only and not notarized)
e. 5th Company: Offer letter, Confirmation letter, Relieving letter, pay slips of till last month and Job description which has all duties (I had signed this job description while taking the post). Except Job description all documents are in original letter head. However the company does not provide the reference letter as specified in document checklist. (All photocopies only and not notarized).
17. Educational Credential Assessment (ECA): Original copy
18. Proof of relationship in Canada for 5 points
a. Photocopy of proof of Relationship document from Municipality office.
b. Photocopy of Citizenship card of Canada.
c. Photocopy Utility bills and property tax to show the relative is physically present in Canada.
19. Settlement Fund:
a. Balance certificate from bank
b. Statement of investment in stock market
c. Provident Fund statement from EPFO.
20. Identity and Civil Status Document:
a. Birth Certificate: (For me and my child: BC is in English so I have not done notarized), however BC of my wife is in both Hindi and English (i.e. its bilingual). I am planning to get it translated and notarized. Please advise me if it is required.
b. Marriage certificate
c. Please let me know if any other document is required as I have PAN card of India but I am citizen of Nepal. So is it fine to give Indian PAN Card.
d. Please let me know if I can give copy my citizenship card. But the citizenship no. is in two parts e.g. 1234-56789, whereas the English translation of the document shows only one part i.e. 1234. I am thinking not to include PAN card and citizenship card which will ultimately create confusion for CIC guys. Still I seek your valuable suggestion.
21. Children’s Information: photocopy of Birth certificate which is already in English so not notarized.
22. Police Certificate: I will give this document later once I get request from VO.
23. Photo Requirement: As per document checklist
24. Fee payment: Fee Payment Form – Application for Permanent Residence (IMM 5620): Paying by Credit Card
25. Self-addressed mailing labels 2 copies: Both in English

for work experience you could get a email refernce from your previous/current bosses satating your duties from his office email ID with his official signature ( landline number position mobile etc) . get that lettr printed and send as a proof that your duties are matching with the NOC u r applying.. salary slip joining relieving letter are find. i put a cover letter for all my experience explaing what i have included along with period of employmennt

u got a BC in English from Nepal?? . and hows your wife's BC in hindi and english and not in nepali. generally the BC is in nepali which has be to notarized by a authorised notary personnel in Nepal.

PAN card is not necessary as far as i am concerned.

citizenship card is our citizenship identfication number proof which is a mandatory requirement in doc checklist. you must notarize it and send them the copy of it.

21. Children's Information: photocopy of Birth certificate which is already in English so not notarized.

1. I will try to get the reference letter from my previous boss from his official e-mail ID if possible. Elease I will attach the job description and mentioned the job duties in details in extra sheet as the space was not enough Schedue 3e form.
2. Yes, the BC we got from Nepal is in english too. Actually I received 2 copies, both original, one in Nepali and one in English.
3. Since my wife is from India, the birth certificate contains both english and hindi fonts, whereas the details are filled in English. It is exactly similar to passport, where the field is written in Nepali or Hindi and English whereas the details (e.g. our name) will be printed in English. Similarly we do not notarize the passport too.... Please suggest if BC for my wife needs to be notarized. Will it be translated document or affidavit or notarized and what is the difference. Also the details filled in BC are hand written and not computerized printout....
4. I agree to enclose the copy citizenship card and original notarized (translated) copy, however the citizen ship no. is mismatch in passport and the notarized copy. Since it was notarized in nepal, I do not have option to get it correct as I am in Bangalore now and cant go to Kathmandu....
If this is fine considering minor issue, I will attach, else I will have to escape this document.

i think u need to get ur notarized copy of citizenship card corrected . contact some relative of yours back in kathamandu who can help.

whiich NOC are u trying to apply. u say you are in IT. IT NOCs are almost all capped by now.
