Dear Babak,
Yes, You need to fill UCI, date of birth, Address, PIN Code and Phone Number (Mobile Number) then send the original to VO along with e-medical information sheet, passport copies and put request letter on top which was attached in RPRF email so that they can immediately scan the bar code of your application and make the entry in their portal for receipt of documents.

Thanks for replying,

This means that the name in the table should be my name instead of my friend's?

The postal code has place for 6 characters while postal codes in Iran have 10 digits.

Area code should be +98 or 0098 for Iran's code?

Hello all,
I have a situation and don't know how to handle it
I got passport request from LVO but the problem is they told me to submit the PP in Riyadh Visa office. Since i have left Saudi Arabia on final Exit and currently in Pakistan. I will be moving to Australia permanently on 26th March. Now the problem is i cannot take the risk of submitting my passport in in any visa center in pakistan neither i can submit it in Riyadh.
I want to request LVO to extend my Passport submission date and change my visa office to Sydney-Australia.
How to request LVO, i mailed them twice last month but no response. Can anyone please advise how to contact them???

Hello Everyone,

I received my MR+PCC+RPRF mail from Delhi immigration on 14th March 2014 and in that email I was told IN bold letters that a separate email with detail procedure on medicals will be sent within 48 hours.

I waited for 48 hours but did not get any detailed email regarding medicals from NDVO. i checked junk/spam folders as well. So as soon as 48 hours passed I sent an email reminding them to send me medical forms/detail email but till date(i.e. a week after MR request) there has been no response. In original email I had three attachments- one letter requesting to undertake medicals, pay RPRF and provide PCC and remaining two attachments were pertaining to CIIP.

I have not received IMM1017 form though have medical request letter with me. Please advise what should be next steps from my side. Has any one else also faced similar situation. SHOULD I CONTINUE TO WAIT UNTIL NDVO SENDS IMM1017 FORMS OR SHOULD I JUST PROCEED WITH MEDICALS?

I just received mine...Please check your email. These forms come from Client-update-mise-a-jour @ cic.gc.ca

Good news and I can't contain this JOY. Its too much and what can I say!

My God have been faithful, gracious and kind. Never thought I would have the opportunity for FSW1 for me and my family.
But now the journey is almost complete. Yesterday, I got the Ready For Visa email and this morning my ECAS was updated to Decision Made.

In this forum, you guys rock. I have enjoyed every bits. One thing is clear, we will all get there guys. Your RFV is coming.
Thanks a million.

Besides Passports, are we also required to send passport photos? Nothing like that in my email. What do you think?
Thanks

Thanks Avinir :) +1 to you

One more question please

How does they do employment verification? Visiting or Telephonic?

I have to confirm my previous employer about it to get rid off unnecessary delay or trouble.

Due to lots application burden. Your file is transferred to Colombo office. All the PER received in 2015 will have VO CVO or warsaw. Its normal. Now you have got the AOR mail means they will start processing you application. This will not be much of delay. Put your fingers crossed for interview. 1% of candidates are called for interview. I don't think interview will be scheduled in CVO. Let seniors put more light on Interview location if VO changed from home country to another country.

How about you contact the closest doctor's office to your location and ask them personally - that's the fastest way to get your answer

You may use the following link:
http://www.immi.gov.au/gateways/panel_doctors/emedical/#c.
Though the above link is for Australian immigration, most of the hospitals in the list are approved by CIC also. In the above link read the section "How will clients know what clinics are eMedical enabled?". If you find any hospital that is common here and in CIC list, then you know that the concerned hospital is e-medical enabled.
