Hi Ankush,
1) If the clinic for meds is an eClinic, then it will be uploaded by them directly. They will give you an ack and receipt of payment. While not necessary, you can share a copy of your ack along. (No Attestation needed)
2) Fill in the blank spots in the RPRF receipt and share. No attestation needed.
3) For India, you just need one PCC. The one you get from Patiala should be enough.
4) Share normal passport bio pages copies.
Also attach along your MR, RPRF email where these specifics were asked for.
Use Bluedart. Better than Indian post, at later stage you can also get a proof of delivery from them. If they have issues with the PO BOX mentioned in the address, you can skip that line and sent it to VO.

As I communicated earlier that I received the MR,PCC and RPRF request on Thursday. Initially I applied throught a consultant but now I feel that I can take it forward myself. Just need a little guidance from all the seniors here:
Medical: Complete or provide proof of completion of the Immigration medical Examination to be completed by you. This must be received at this office by: 2015/04/13
Currently I am in Kolkata and I have taken 20/03/2015 appointment with the doctor. Do I Need to send the receipt or medical results or will the doctor upload himself on CIC website?
RPRF: Pay the Right of Permanent Residence fee for yourself. This must be received at this office by: 2015/04/13.
I have paid the fee online and have taken a printout. Do I need to send the receipt. Do I need to get the receipt attested?
Police Certificate: From each country/state where the residence period has been six months or longer since the age of 18, provide an original police clearance certificate for you from India. This must be received at this office by: 2015/04/13
As my permanent address in Patiala thus I have applied for PCC (Police Clearance Certificate) there. Do I need to get another Police Certificate from Kolkata as I have been staying here since a year or will a single certificate from Patiala solve the purpose? Is there any particular format (to be downloaded from CIC Website) or will the certificate issued by police department solve the purpose?
Passport Photocopies: Do I need to share the attested photocopies or just the normal photocopies?
Apart from these documents do I need to send anything else to the immigration office?
Should I send using a courier service or Indian Postal service?
Kindly help me with these questions as I am really worried if I will be able to send the documents properly.
