I have come across the similar situation as they requested me to submit PoF with my MR.

I had submitted Bank Balance Certificate, FD and EPF notorized

The med officer didnt ask in me and my families case. It all depends to each officer. However the questions asked are a routine.
If asked and as you said, you are fully cured. I think you can skip the answer. As the gland was in neck and the x-ray is chest, I suspect they will be able to establish any connection.

and if so then I lost my medical reports for this long ago so I don't have any proof to show them
then what will they do? is there any chance that they will reject my file due to this????
 
DD is presented to bank within a week usually from being received at NDVO.

COPR and passport with VISA are received together.
Each passport holder get's 2 COPR which are bigger in size then a A4 paper and a tad bit smaller then A3.
Out of the 2, one COPR has you photo and the other doesnt. The one with photo is what the IO will take when you land, towards you PR Card application.

This is not true.. its a myth created by some forum members only.... Every case depend on visa officers working capabilities and work load.. You can see May and June applicants are still waiting for MR and July,August are getting PPR's
 
Exhaustive list would be better.
Same for the list of item's you will get at a later date.
