I dont know the latest but i know someone who got per, a sep12applicant for noc 3012.

I put following:
1. Letter which was sent by CIC
2. DD
3. Proof of medicals
4. PCC
5. Passport front & Back

I am not sure if they accept in person. I sent it by DHL. It reached next day.
