In "Additional Family Information" File:- IMM5406E.pdf

Whether I have to fill separately this form and sign it?

And another copy of this same form will be filled by my wife and she should also sign it?

Means there will be two copies of this form one signed by my wife and one by myself?
You're absolutely right that you'll fill two copies one for you and one for your spouse.

Your police clearance certificate is valid for a period of one year from the date it was issued. If the validity has expired before your visa is issued you will be requested to provide an updated police certificate from the country you are currently living in. If you have already submitted police certificates from other countries where you have not recently lived you will not have to provide an update

thanks sunnyvin for your reponse... however, you did not understand my question. i have submitted, along with my app, pcs from the countries where i lived and live in the past 10 years. however, i did not submit pc form the usa, where i've lived before 2003., which is 11 years ago. my question is: do you guys think that LO will ask for the usa pc as well, since i was over 18 years old when i've lived there (but over 10 years ago)? i hope my question is clearer now :)

In the Document Checklist they are mentioning that LEAVE DOCUMENTS UNBOUND.

So it means that I should not staple the relevant documents. For example Generic form is consisted of 9-pages should I not staple these nine pages and LEAVE THEM UNBOUND as they mentioned.

Similarly my birth certificate is in Arabic Language, which means that CIO officers will not be able to properly recognize it and pt]SHOULD I NOT STAPLE THE COPY OF THIS BIRTH CERTIFICATE IN ARABIC LANGUGAE WITH IT'S TRANSLATAION. should I leave it unbound? Because I think if officer reviewing my file does not know arabic letters he will not be able to recognize that it is my birth certificate copy in original language. Although translation copy will be there in envelope.

Simlarly other relevant documents. Please advise.

George

As i have received call from unknown number however i was not able to pick up that call. kindly let me know, anyone in this forum has received call from CIC after DD charged and before PER.

george, you understood correctly. DO NOT staple any of the docs. docs which have more that one page, you can gather and bound with paper clip. do not staple any of it. cheers!

Yups, that is fine. It might be valid for one year but that is when your application is received by CIC. As per the CIC website:

Who needs a police certificate?
In general, you and all the people in your family who are 18 or older need to get a police certificate. You must get one from each country or territory where you have lived for six or more months in a row since the age of 18.

The certificate must be issued no more than three months before you apply.

If your certificate is in a language other than English or French, send it when you apply, along with the original copy of a translation done by a certified translator.
See http://www.cic.gc.ca/english/information/security/police-cert/intro.asp
This is applicable in case you are submitting PCC along with application to CIC.For visa office submission arrange a PCC after getting PR and the PCC is valid for one year.
