Certified or notorised copies are only required for the docs which are not in English or French. Normal copies of docs which are in English are just fine.

LAST NAME :

hOW SHOULD I WRITE MY NAME IN pr APPLICATION..... LAST NAME COLUMN IS COMPULSORY FIELD....GIVEN NAME IS WRITTEN TO BE SAME AS ON PASSPORT.................

i am 18/08 NOC1113 applicant i had sent some additional docs to cio . i called many time to cio to enquire whether these docs are attached with my earlier package .but they told me they cant provide any information for f s w . can any one suggest me contact no of cio and direct extn no for F S W P .EALIER I CALLED ON 0016139444000AND THEN 1,1,1,13, AND THEN 1 . i am 18/08 my file is already done with completeness check . i alredy sent them mail on 21 october no reply .

Calm down my friend. You have done your part, cio will not provide any confirmation like this. You should write an email to inquire the status and should expect reply after 45 days. Hopefully if your addendum is attached and you will get PER before the reply of your mail.

Today I got reply (after 18 days of receiving PER) for my query regarding addendum inclusion. But still inclusion was not confirmed only transfer of my fileto vo was mentioned.

Just one more question, by certified you mean you have them notarized? Well, for the documents coming from Canada (from the relative) do those need to be certified? If yes then from where? from Canada i guess....

I have couple of questions regarding PCC application in USA.
(1) Do I need a cover letter or any other additional documents other than I-783, Fingerprint card, and money order?
(2) I have two separate applications, one for me and another for my spouse. Can both applications be mailed to FBI together in the same envelope?
