Yes. It is mandatory to provide all the listed information and documents about your children unless the Guide or Document Checklist say that particular information or document is not necessary for non-accompanying dependants. It is especially important to provide the information if you wish to sponsor him to come to Canada someday, but even if you don't wish to do so it is still required. If it is impossible to provide this information you can explain it in your application, but "I haven't applied for the passport" is not a good excuse.

Well, they require only stuff that is mentioned in the checklist, so if you are provided accordingly, your boat is still floating :)

If there is important information on the form that does not appear in English or French, then yes it must be translated. Date of birth is certainly important information.
 
As long as you correctly put NOC code and description at Q 12 of Schedule 3, don't need to be worry.

If FedEx shows that, you can be sure it was delivered. CIC will not acknowledge it. The first communication you get from them will be PER.

facts about my application:

1) Stapled each form
2) included 1 year 3 months exp of articleship (Total articleship 3.5 years, however managed to get reference letter of just last articleship) (Duties were matching 70 to 80%)
3) 8 Months in Company A(Duties were matching 70 to 80%)
4) 2.5 years in company B( Duties were matching 50 %)

Self calculated points: 70
out of which 13 is of experience

Now, what is the possibility from above, they have considered either articleship experience or experience in Company B.
Other possibility is that they have considered both experience.

Unfortunately he cannot receive credit for his time as a chef. He can get credit for any experience at skill type 0 or skill level A or B, even if not related to his primary NOC, but chef is at skill level D and cannot receive credit even if it is related to/in the same field as his primary NOC. He can simply explain on the application that a letter could not be obtained. I wouldn't spend too much time trying to get a letter from anyone from that job because it is not worth points.

I think he can claim his experience as a chef as well, because in Canada chef is a skilled occupation and falls under the technical level B. Even cook is a level B occupation. The second digit in NOC determines the skill level. 2 and 3 are technical skill B, anything more than 3 is not qualified as skilled.

I found one error. My Name has two Parts - (say) A B. I filled in all my forms - Family Name: B and Given Name(s): A, as I was adapt to it since childhood. But by very first time I gone through my Passport with this scrutiny it has Family Name: (Null) and Given Name(s): A B.

I am worried... Would this impact my application?
