They do another eligibility review in VO, there are different statuses for that in the ECAS:

Application Received by Citizenship and Immigration Canada (CIC)
We have received your application. After checking for completeness, a file has been created and the information from the application form was entered.

COMPLETE

(This is the last step in the processing of your application.)

Applicant Arrives in Canada and is Granted Permanent Residence
You will need to meet with an Immigration Officer who will verify your documents and grant you permanent residence if all is in order.

Not just click that... In that way we can also know how our post was not helpful and we can improve.... So don't be a hypocrite and be brave.

Absolutely. It is unfair on us to see -1s being awarded in spite of our best efforts to help and dont know who has given it and for what reason. And these are deeds of cowards, although it does not make an iota of difference for us.

Guys, don't do the mistake of sending only one FD-258 Card. At-least send 3 copies for your application. This is because, if the finger print is not clear on one card, FBI can use the second or the third one, until they finally feel satisfied.

More than 3 would just be too much, so please don't exceed that.

My gut feeling is that medical request means either background check has been done or it has been waived.

There is nothing significant to "Start processing...." with respect to background checking. Everybody will have this stage.

I agree with you Century. There are many applicants whose Ecas status is "Started Processing" but they have not received Medical Requests. I think all those applicants whose files are very clear and does not require verification gets MR within few days of ECAS update or sometime even before ECAS update. If applicants for whom VO thinks that further verification is required or they may be selected in random selection for verification, required to wait little more. I request to all those applicants to chill, if their documents and experience is genuine they are definitely going to get it sooner or later. After all they need to clear Medical Exam so don't raise blood pressure by getting tense.......lol........
