this is fine for now. When the VO contacts you for MR/PCC or anything else after PER, you can update them about this. Sorry about the accident, i hope she is doing well now.
 
Yes, you should inform but not now. Once you get your PER, inform your VO about it.

no, for India just one PCC for the current residing address.
 
Yes brother. All the country except Canada - if you stay 6 months or more and you were 18+ years old.

Kapoor, I submitted my wife's matriculation certificate as Birth proof although her parents name is not there. So I made affidavit with her parents name in it mentioning her date of birth and sent it along with matriculation certificate. Guess what....I got my PER!!

Dont forget to mention 'Birth proof' written on the top of notarized matriculation certificate and on original affidavit.

Can someone pls provide ICICI Bank Phone Number/Email ID which can provide status of Foreign Currency Demand Draft ??? I think usual customer care people will not be able to give this info.

BTW, I called Bank Of Montreal on their number which is printed on DD. The lady there told me tht she can provide the status only if my local bank calls/writes to her.

To Answer Your 4th Question, Your Indian Passport will be Stamped with PR of Canada and after 2 Years stay in Canada you can apply for Citizenship only then you will get your Canadain Passport but your Indian Passport will be Cancelled as you cannot have Dual Citizenship and you need to get a Visa to come to India, Others Please Correct me if i am wrong.

Still did not hear any news about PER, As per bank update my dd was encashed on 22nd Aug and my file reached on 14th May. I can't really stop thinking of the part why its getting delay. As your DD is already en-cashed, i think your PER should be just on the way, unless they are not able to get 67 points from your application.
1. Have you added your email for correspondence. If not they must be sending using regular email.

Also you can drop a mail to CIC asking for status update... they should reply in about 20 to 30 days i think.
