Don't worry sister. If I get PER with the old form, why you won't get with the same though at that time it was changed. Consider people who sent application through normal post whose file received after 10-25 days of sending and in between forms have been changed.

For peace of mind, do send a revised and updated forms with cover letter about the changes.

Thanks Century for your re-assurance... But i am being a skeptic here :( especially after seeing in 2013 forum spreadsheet, that 10 applications were reported to be rejected due to submission of old "Schedule 3 Form" :'(
Not sure what was the change in their new schedule 3 form then? it could be a major change, resulting in rejection, but without that data in hand, i think its tough for me to relax :(

I will start working on sending the addendum. Could you again please provide me the procedure for this? also i believe it should be sent to the same address where the app was sent? correct me if i am wrong....

I just got my PCC from Police station in Delhi. Here are the contents, can anyone confirm if it will work:
1. it is on letterhead on "Office of the Deputy Commissioner of Police: Delhi Police" withtheir logo at top.
2. Content states that "Vishal Bakshi son/of ... Living at address "....." is not involved in any criminal offence as per record of Police station in which his residence is located."
3. name and Signature of ACP

Can someone confirm this is what their local PCC looked like?

In my understanding the question addresses two things:
1) conviction
2) whether I am currently subject to any criminal proceedings.

Should I answer Yes or No to this question?

My lawyer says that I should select Yes and proceed with explanation.

But I am inclined to select No and then, provide explanation below stating sth like " I have been charged on such date with such an offence and it was dismissed ...etc". But if I select no and the charge appears on PCC, isn't it going to be an issue with the visa office?

What do yo think and suggest on my situation?

It may be useful to obtain a letter from the U.S police or FBI exonerating you from above charges, which you could attach to your application to explain "Yes" response to 6b.

Don't worry at all, won't be an issue !!
Using a "zero" instead of a "definite" article does not lead to any ambiguity to whom to encash your DD.
 
The difference between 2013 and 2014 form is wholesale and retail changes. For any wholesale changes, there was 100% rejection (you can say wholesale rejection). For retail changes, no rejection.

Did you heard anybody rejected due to change of Additional Family Information [IMM 5406] (PDF, 570 KB) on September 2013?

Send the updated forms with cover letter to the same address.
