In the main form IMM 0008, In "consent to release information" Section I mentioned my name as KAZI ISLAM but my full name is KAZI ZAHIRUL ISLAM. I did it as they mentioned to provide (first name, last name). Will it be a problem not mentioning the middle name here?

It will not be a major issue....Thats what I think.....

Just for ur info that when there is no middle name then middle name will be counted as first name....

I think you will fill up as:
From.... To Give joining date and date of signing
Country and level of jurisdiction: Regional
Department/branch: Don't know? Health Cadre???
Position Held: Doctor

My wife is a teacher in MCD (Delhi Govt) but I did not mention anything in her background declaration form Q10 (mention any govt. positions held) ...now I am confused if I did it right or wrong...?

As a teacher, your wife is just a government worker, not in a government position like elected councillor or appointee. Relax, all is well

Thanks for your reply. I know many people in India who can endorse me for UK PCC but i thought endorser should be from UK?

When did you apply for UK PCC and when u received it?

I applied in April and it took about 10 days
