Hi,

I have one query regarding the "ADDITIONAL FAMILY INFORMATION" FORM (http://www.cic.gc.ca/english/pdf/kits/forms/IMM5406E.pdf).

1. My father's name in passport is Chacko. This is the name given in most of the records excluding the birth certificate, in that it is given with initial T.C Chacko for this I have affidavit. T.C is having an expansion (Expansion is not mentioned in affidavit). In the "ADDITIONAL FAMILY INFORMATION" I have given the name with this expansion, but it is not there anywhere. Will it be a problem, or I will simply give like in the passport (Chacko).

2. Same issue in mother's name. Mother's name is just her name in passport. But in the form last name is added with father's name. Please advise me on this. Do I need to make any change in the form.

Dear Seniors

Request for advice. My spouse's birth certificate has her parent's name at the time of her birth. Since then, they changed their names through a notary affidavit, advt in newspapers and publishing in state gazette.
New names are used in her passport too.
I have a copy of the advt and gazette notification but the notary original was submitted to the government and no copy was made.

Can I attach the advt copies, gazette notification and explain the case to CIC while submitting the application? or how should I go about it?

Hello All,

So the final package is ready today, just a couple of things pending which will be completed by tomorrow and should be sent by tomorrow's courier if all goes fine and as per plan. Below is a brief on the final set of documents, would request seniors, experts for their views and comments for any last minute changes so that I may reduce any chance still lying for my application to be returned back

7. Saperate Declaration for Minors: Not required (Still printed, hand crossed as not applicable) Anything else needed?
8. Statutory Declaration of Common Law Union: Not Applicable (Still printed, hand crossed as not applicable) Anything else needed?
9. Fee Payment Form: Completed with credit card details, printed and signed
10. Use of representative: Completed, no representative required.
11. Document Checklist: No documents notorised unless specified below

1. Passport Copy: First and Last Page --> Black & White Print
2. Current Employer (4 years 3 months) (4 years relavant for NOC 1113, Rest 0122): Offer Letter, Salary Slips since Jan 2013, Promotion Letter, Reference Letter on company letter head signed with phone no and email id of supervisor (will get this tomorrow), last 2 years tax statements, last 2 years form 16. Anything else required?
3. Previous Employer (2 yrs 6 months) (NOC 0122): Offer letter, relieving letter, Salary Slips, PF statement, Form 16, Self affidvit for job reference (Notorised) on Rs 10 stamp paper. Anything else required?
4. Previous Employer (1 Year 4 months) (NOC 0122): Offer letter, relieving letter, Salary Slips, Self affidvit for job reference (Notorised) on Rs 10 stamp paper. Anything else required?
5. Previous Employer (9 Months) : Only offer lettter and reliving letter, no other document available, not claiming points for this job as well
6. Education: Original ECA Certificate, copy of degree and marksheets (not attested by registrar), 10th & 12th marksheets and certificates. Anything else required?
7. Original IELTS Scorecard: Total score 7, Speaking: 7.5, Writing: 6.5, Listning: 7, Reading: 6 Shocked Shocked (I dont know why reading so low)
8. Birth certificate: NOT AVAILABLE, 10th marksheet with declaration from mother (Notorised) & on Rs. 10 stamp paper
9. Funds Proof: Provident Fund Statement from Employer + 2 Insurance investment documents clearly showing surrender value as on date + Account balance certificate (I am Joint account holder with father & grand mother [Primary account holder]) any issues here? I can give balance certificate of another account but only has short term funds and will be available for short term only

Thats all as of now... request as much suggestions on this as possible so that I may get the documentation write in first shot with minimum chances of return.

I am really worried for this documentation, an early response will be much appreciated.

Good day , I'm going to apply next week ( NOC 2281 ) inshallah . I have a query about job duties . I'm serving my company since 2005 and still present . During my service period I got 02 promotion and currently my position is IT Manager . My job description letter explain all my duties and time frame for each position but only job hours, and salary mention about my current position previous 02 position not mention any job hours ( weekly/monthly ) and salary details , Does it any problem ?

Is it enough if I have my Reference letter with all the details like Job description, Salary, Position and Duration but i dont have any other attached documents like payslip and etc., only just one reference letter i will submit.

Please advise.

Ideally yes it is enough... But it is always good to attach supporting documents like offer letter, pay stubs etc.
If they are not available even after your 100% effort then go ahead and submit with just the reference letter.

Can you please tell me step by step what i need to enter ? If someone does not have UCI number is there any provision to know ECAS status ?

Also does it mean that they have rejected my file because there is no ECAS update ??
