What if you do otherwise.. I mean you can make an affidavit that you are changing your last name from _____ (BC Name) to _____ (PP Name).. That clears the matter.

Just wait for another email -AOR email from NDVO....Then u can send ur PCCs by courier to NDVO....:)

Can anybody give me affifavit format for the same???

AOR emails differ from Local VO to VO...

So Singapore has one format...NDVO has another .....

PLEASE DO NOT USE MAR 2014 FORM....

In 2013 my application reached @ CIC on 4th July and they updated "Additional Family form" on Sep 2013. However in my rejection letter along with my other mistake, they marked i did not provide my updated "Additional Family form" form.

Bottom line is as long they have shared a new form and you have time in hand, please print the updated one and send it to CIC. Rest upon you...

Like kollins said, the choice to tell them later is yours. We can't tell you what the VO will do but it's unlikely your bank will divulge this privileged information.
