As you are giving proper documents, visiting card is not required - I guess, in the reference letter, your supervisor's contact address has been given.

You can add separate page for each section of document checklist; not required against each documents - because those are in English and can understand by reading.

Also note about translation:
1. Translation must be done by certified translator and need the translation copy notarized
2. Photocopy of original document need to be notarized;
3. Translator must give affidavit.

Thanks Century, it helps a lot. +1 for you. well there is full address of my supervisor including his email, cell no etc. Regarding affidavit the translator has given me his licensed copy that was given him by Law council (just like Engineering Council for Engineers) where he is authorized to notarize and or translate documents. I hope this could be treated as Affidavit.

Extremely sorry for hurting you :-[ :-[ :-[. I know, you did not received medical request. Now just realize Passport need to be sent after medical request.

For temporary visitor visa, passport need to be sent through vfsglobal at Gulshan; later on, medical request is issued. I messed up thing with permanent visa and temporary visa.

My apology again. :( :( :(
 
I am from India .
I called psk helpline no ..they Only told that you will be issued new passport..as this is a passport request .

Regarding the process I checked online for the list of documents ..it looks simple.. I don't know the ground realities ..

They did this for him.

Are you from india?
Have you check the website of passport office for change of place of birth.
It is not easy to get the place of birth chenged.

any senior can advice that whether you should go for this or not.
 
Dear Friends,

I have an issue with my spouse passport, For myself and son Passport valid for 10 years, but for my spouse its expiring on 2016 June.
Is that an issue?, I heard VO issue PR depends upon the validity of the passport.

Is it possible to update a new passport to VO? if how ?

There auto generated mail states follow 8 to 10 weeks after sending the Apps.

But as I know you can follow up your application if 6 weeks or more with no feedback from them.
