Nope, not a problem. For me (as a single applicant) it generated a set of 10 barcodes.

also when I print the form the city gets changed from toronto to a number 3812. anyone faced this thing?
Never faced such thing. Trying changing it to some other city & then back to Toronto, see if it helps.

No....you are fine.
Just use the latest generated bar codes forms in your submission.

As a citizen of Nigeria, you always have the option of indicating that you want your application processed at the Canadian visa office in Accra, Ghana (which is responsible for processing permanent residence applications from Nigeria). You also have the option of selecting the visa office in Sydney, Australia, because you have been granted legal status in Australia for more than 1 year and you are currently living there. It does not matter that you have not lived in Australia for one year.

• You are a citizen of India who initially entered the United States on an H1-B work permit valid for two years. After your work permit expired, you remained in the US without valid status and never left the country. As usual, you can select the visa office in New Delhi, India for the processing of your application. Given your circumstances, you can also choose the visa office in Buffalo, New York. This is because you were initially granted legal immigration status in the United States for more than 1 year and you never left the country.

• You are Jordanian national who was admitted to the United States on a visitor visa for 6 months. After your visa expired, you left the US for 2 months. You have now come back and again you have been admitted for 6 months on a visitor visa. You cannot have your application for permanent residence processed at the visa office in Buffalo, because you have not been granted 1 year of continuous legal status in the US. You can only choose to have your application processed at the visa office in Damascus, which is responsible for processing applications for Jordanian nationals.

I don't have any gaps because on the declaration the persoanl history section i wrote from sept 2004 - sep 2006 i was studying, as a student the university allows u to work and travel for the summer which i did and was therefore living/working out of my home country, so for the last part of the form, it asks to list all the address you have lived since your 18th birthday, so thats the section I wanted to know if I had to put the address for where i was living for the summer

But in this case you will have a discrepancy between the various forms. You do need to enter where you were living during the summer. You should also enter your work in personal history. I entered summer work in mine. I only made one entry for the school but filled in additional lines for the summer work. I suggest not leaving anything you did/lived/worked for as long as 4 months off the forms.

I think document checklist should be on top and then barcode forms.

It varies from country to country. For India, attested copy of marksheet from registrar or EOC is required in university sealed envelop. And yes, you just need to put passed marksheet.
