The best way to possibly do this if they refuse to give another original copy is to take all your application package to the agency for them to insert the original copy of the IELTS for you and send it on your behalf to CIC.You will pay the courier charge to CIC for the agency.
Trust me....it works as I'm advising you based on my personal experience with British council .

That's normal. Even mine online report doesn't show name and Dob, but actual hard copy had all.

Your passport is expired already......You need a VALID AND CURRENT PASSPORT to apply for Quebec or Federal skilled.
Your expired passport ID is null and void.

Three things is very important for birth certificate:
1. Name of the person who is born;
2. Name the parents
3. Date of birth

And also important is place of birth.

My birth certificate has my name and my fathers name. They said they dont put mothers name. Do i need to make an affidavit for that.

Do we have any link/URL on CIC site that gives us some info; what one needs to submit if any of the companies closed their businesses ....

2.Notarized the translated copy and translation done by certified translator This should be on the Translators letterhead right and stamped by him?
3.Affidavit of the translatorAnyone has ever shared of format of this, or is one available on CIC website as guidelines/instructions? What should be really sworn in this document?Does it have have to be signed by Translator and stamped by notary public?

Of all the 3 documents, which one has to go in original apart from the affidavit?

For translations is it necessary for the translator to submit an affidavit or a letter from them will do.
 
It will take a hell lot of time.. for bank's reconciliation to be done at home branch.
What country you from ?

Why 2 DD, is it a problem if I use one CAD1100 for me and my wife? as we are sending one application deck.
Different applicants with their opinion.
2 DDs may work for others even though I dont really see any logic behind multiple DDs in the same application.Its just like give out several details of CC in one application.
The fee form already have a summary of the applicants processing fee so why create unnecessary bottlenecks with multiple DDs?
Imagine a family of enclosing 5 DDs inside one application package....what impression are you creating in the mind og the CIC officers handling your package?
Anyway,I applied as a married applicant and I raised only one DD for the three (3) of us...and it was successful.
My advice....only one DD is enough....especially when it was not stated out in the instruction guide.

I have applied for WES and my documents already reached to WES office yesterday but still i am seeing message 'My documents not reached'.
