1. Where do we place it in the application package. At the front, or in between wherever applicable?
2. I am not sending my PCC now, but i have applied for the same. Should i mention this in the covering letter?
3. I got a rejection letter last year due to cap fill, should i mention that in the covering letter?

1)Where applicable.
2)Mention it and state the reason for the non-inclusion.
3)You dont need to mention it....even if you mention it,it doesn't really matter.
 
Is Cover letter mandatory to give?

I did not provide any cover letter last year.

It may just be a typo error.
NOC stands for National Occupation Classification.
Each occupation is classified differently and the only way to denote is by assigning numerical codes hence the 2281.

They are quoting it as a 3-4 months long procedure. So waiting for the new forms and probably some more updated info about where to get ECA for doctors. If they dont release today then tomorrow is May Day and dont expect an update from them on a holiday.

Please remember that MAY 1 is not holiday in CANADA. They celebrate Labour day in September. SO keep fingers crossed and wait. Hold your horses guys. It is only less than a day to send the prepared documents.

In 2013 the processing started not so fast as it was the first time we needed to get ECA. But now people like me who missed last time are almost ready and we have room for more people this time as from 300 the CAP is raised to 1000 in each occupation. So take time and check thrice all your details.

Any idea when would the new application guide and forms would be released?

I believe they were supposed to release the new instructions on 26 April but i do not find any new notification so far

Can someone post the Canada Gazette link where i can check for notifications regularly??

It depends on the way you see it.
Just to be on a safe side....whatever documents like PCC that you cant include in your package,you may write a cover letter to explain the non-inclusion.
Cover letter may be useless in some situations.....trying to explain with cover letter why you did not include ECA report or IELTS result is useless as they will not even attend to your package once they discover its missing.
