You explanation explains the situation and it should be acceptable by VO.
In my opinion, they have asked you to submit within 30 days than information that you will initiate. Therefore, considering the delay of mailing to VO, you may submit the cover letter answering their questions and also bank statements, payslips, approved leave application and PCC in one shot.
The response may look like this:

With reference to the letter received from your office dated xxxxx, kindly find below my responses:
1 - The schedule A/background forms have been completed and enclosed with this letter.
2 - your answer is appropriate
3 - your answer is appropriate
4 - Payslips and corresponding bank statements have been attached with this letter.
5 - Maternity leave started from xxx to xxxx for xxx days. It has been fully/partially paid and I have attached the approved maternity leave form as well as corresponding payment in the bank statement/payslip has been attached.
6 - I have arranged PCC and these have been attached with this letter.

The mails says the below:
(a) New Schedule A/Background Declaration form for yourself and your spouse since the age of 18. This form must be fully complete. Each month must be accounted for and listed
(b) Explanation for your change of name
(c) Explanation as to why your marriage was registered in Nov/02 when your marriage was solemnised in Mar/01
(d) 6 months of payslips and corresponding bank statements showing salary deposits.

(f) original police certificates for you and your spouse

(b) It is tradition for Hindu female to change her name at the time of marriage
(c) Registration of marriage is not compulsory as such in India at the time of marriage. At the time of application for passport for the first time, marriage certificate was taken.
(d) Maternity leave certificate from company will be attached
(e) Action to arrange Police Clearnce Certificate will be initiated
