1. "Alternatives to birth certificate are as follows in CIC's own words

We do accept Secondary school records that contain the following :

Applicant's Name
Date of Birth
Name of Parent/Parents

If you are not able to provide a Birth certificate with all 3 criteria, we require an affidavit stating all of this information or that a Birth certificate cannot be attained.

So 10th standard school certificate or an affidavit containing applicants name, date of birth and Parent/Parents name should be fine in lieu of a birth certificate. Also you must give reason in a cover letter why you were not able to provide birth certificate and that you are providing 10th standard school certificate / affidavit in lieu of birth certificate."

2. Not acceptable.

3. If you contact any notary public, they will readily supply the format to you. However, the following are for only reference purpose:

AFFIDAVIT​

We, Fathers Full name and Mothers full name " (Maiden Name) being duly sworn, hereby affirm and state:

1. We reside at "full address including country name".

2. "Father's First Last Name" was born in "City, State, Country," on "Day Month Year".

3. "Mother's First Last Name" was born in "City, State, Country," on "Day Month Year".

4. We have been married since "Day Month Year".

5. "Name of the person for whom this affidavit is being made" is our "son/daughter" who was born on "Day Month Year" in "City, State, Country".

So I do hereby solemnly affirm the foregoing are true and correct statements.

I am a 15th May Applicant under NOC 1112. My DD is still not encashed, but when I check my application status in CIC website with my UCI of last year, the status shows in Progress and "We received your application for permanent residence on May 15, 2014."

PER mail not received till date.

Any advise from our experts who have faced such situation.

Apart from tracking number, is there any way to know if my file hs been reached CIC? I am asking this because my consultant is not sharing the tracking number. He is just saying the documents are mailed to Canada. Dont understand how to track my application.

Well you must have tracking number to track your package. Maybe Pay some more money to ask your tracking number
