I need your opinion about "Additional family info" form.

I am the main applicant and when I fill in the form I write my name in "Applicant" field.
But what my spouse should write in that field? Should the spouse write HER name as applicant in this form? Or it still refers to the main applicant?

Hello Everyone,

I am finally applying for FSW 2014 under NOC 1123 (Professional occupations in advertising, marketing and public relations)

I saw the cic website and only 23 applications approved so far so I guess I am not late yet ;)

I am having few questions/doubts/confusions so help experience people like you will be great!

- My spouse don't have birth certificate, is there any alternate to it? Also his 10th/12th certificate has Jain as last name while passport/pancard and all other documents have Sethia as last name, both are same though but still different. What can be the other option?
- What is National IDs other than Passport? Do we need to provide something like Pan Card? But in the form they asks for expiry date of the National ID as well?

Last but not the least, since I have done Masters in Arts (Media & Production) and was top scorer in the university for both years so got job in my college as Assistant Lecturer immediately. I was there on contract basis (4 hrs per day for 7 months). The contract was renewed for same period for next 3 years.

Now my question is should I apply under NOC 1123 or NOC4011 because my current employer is a small pvt. ltd. company while the other one is College itself which is any day more recognised and preferred. But when I see NOC4011 it all mentions about University and not College, my college is approved by GNDU and all students gets their degrees by GNDU university only. Do I still fall under NOC4011 or I should just focus on NOC1123?

If you had done mistakes in forms and which are not under tolerate level of CIC than probably they had not processed your application. you have sent another courier on 16 june but it is very unlikely that they will consider as a part of your 14 may application. All you have to do now is to wait for positive response. Hope for the best :)

Just a suggestion,

Please use fresh email id for this application instead of using ur regular one
Reason 1: you can count on that inbox for "only" mail from cic in future
Reason:2 cic /lvo won't be able to trace ur fb or linkedin profile linked to ur regular mail id and u can make new fb or linkedin profile with sync to ur application

Precaution is better than cure !

Thank you. The applicant is the person who fills in the form not the main applicant...

Here's an interesting post (from few months ago) from the FSW 2013 thread by an NDVO applicant. The applicant actually met the NDVO official who worked on his file.

My experience at NDVO:

Well I visited NDVO on Monday as needed passport coz spouse had to travel.... My experience as follows:

1. Friday is off for client dealing but the office is open and work goes on inside. So visit days are Mon- Thursday.
2. At the gate its very quite and you would hardly see any people.. It was a surprise to me as I was expecting huge crowd as Canada is a hot destination.. Reason may be they only entertain urgent/emergency cases there rest all have to send mails to NDVO and discuss..
3. Tell gaurds about your urgency request. They will give you a slip to fill explaining your situation and the slip will go inside. 10-15 mins you will be called at the window at the main gate only. The person will discuss your urgency and depending on the case he will email it to visa officer inside and will tell you that the visa officer will decide on the case and he has no jurisdiction to take calls.. he will ask you to wait.
4. After 10-15 mins you will be called at different window next to the earlier one and there you will be greeted by THE VISA OFFICER (you get to meet the guy who is working on your file for almost 7-8 months and who will decide for your future.. My visa officer was a cool guy.. 35 years age.. Looks very much Indian (speaks hindi) though I didn't ask if he is a Canadian citizen of Indian origin.
5. Here you need not worry about being lucky to have a good visa officer as they work by proto-calls and every officer will follow the rules without any exception. If your case has merit you will be entertained.. Well.. he explained me that my visa is already issued and it is a matter of 2-3 days when I will get the stamped passport. I requested him to issue it the same day.. After 10-15 mins of discussion (why I need and how tough it is for him to do that) he agreed and asked me to come @ 3:30 pm.
6. So I was sent inside the embassy @ 3:30 pm and asked to sit at the reception area.. there is pin drop silence in the room.. 4pm he called and gave me Passport and COPR/ILETS/PCC etc. It took just 5 mins but we spent around half an hour talking about visa processing, settling in Canada, do and don'ts etc etc.. I wish I could connect him to this forum so that all the queries get answered. (guy was really gem of a person)
7. He explained how tough it is for them to deal with clients who come through consultants. Really guys if you apply without a consultant it makes the visa officers life very easy..
8. He suggested me that instead of submitting my passport through VFS and sending an urgent request form I should have come directly to the counter and asked for urgent processing (passport could have been stamped in 2 days).

Conclusion:
Like Islandbabe says nothing beats a visit to VO.. I have experienced it myself...
Guys who are working abroad connect to VO by email well in advance they will guide you appropriately and call you only when they can stamp your visa in 2-3 days.. You can even send a relative/representative at the embassy to discuss your case if you feel email is not helping..This way the life of visa officer is easy rather than just landing and informing them to issue you visa soon..
Rest its personal choice to send passport with somebody/take leave/leave job with pros and cons of each.

All the best guys.. hope my experience helps.. I am planning for my travel to Canada.. will keep you posted on that..

When your spouse fills the form they become the applicant and fill the form accordingly. This information is not given on CIC I believe, however I saw a video online advising that; I did the same. Moreover on top of the form it does state that all family members have to fill their own copy.

Q. How should I fill Additional Family Information form for my Spouse? Is it required?

A. Absolutely! It is a required document and your spouse should fill it. The form asks for “Applicant” and not “Principal Applicant“. Take care of it when filling the form. You, as a Principal applicant will write your name in “Applicant” Column and you will write your wife/husbands name in “Spouse” column.

Similarly, when your spouse is filling the form, he/she will write his/her name in the “Applicant” row & your name in the “Spouse“.

For both forms, you will write your kids details [if any], or will sign the declaration at the bottom of second section for no kids.

The last table will be filled separately by the two of you with your own siblings.

yes in case of family information form.. applicant is the person who is filling his or her form.
