Hi Seniors,
I have name difference in my passport and educational document. I searched at few places and came to know that we need to do an affidavit for the same. Can someone please review below format :
AFFIDAVIT
ONE AND THE SAME PERSON
I, the undersigned, Mr. Surname ABC XYZsinh, aged XX years, XX by religion, residing at XX, do hereby solemnly affirm and declare on oath that:
1. I am the deponent herein and a citizen of XX.
2. I was born on Month XX, YEAR in XX, YY, India.
3. My birth name is Surname ABC XYZsinh.
4. It is customary in our region to add the tags such as 'sinh', 'bhai', 'kumar', 'lal' etc. for male first names for showing respect, while to add the tag 'ben' for female first names for the same purpose. This is similar to putting Mr., Mrs., Ms. etc. in front of the names in the U.S.
5. My father's first name was used as my middle name in most of my documents. The tag ‘sinh' or 'bhai' was interchangeably added in my father's first name.
6. Therefore, the following names refer to the one and the same person:
Surname ABC
Surname ABC XYZbhai
Surname ABC XYZsinh
Surname ABC XYZkumar
The above are the variations of the same name and should not be considered name changes.
So I do hereby solemnly affirm that foregoing are true and correct statements.
