Hi Seniors,

I have name difference in my passport and educational document. I searched at few places and came to know that we need to do an affidavit for the same. Can someone please review below format :

AFFIDAVIT
ONE AND THE SAME PERSON
I, the undersigned, Mr. Surname ABC XYZsinh, aged XX years, XX by religion, residing at XX, do hereby solemnly affirm and declare on oath that:
1. I am the deponent herein and a citizen of XX.
2. I was born on Month XX, YEAR in XX, YY, India.
3. My birth name is Surname ABC XYZsinh.
4. It is customary in our region to add the tags such as 'sinh', 'bhai', 'kumar', 'lal' etc. for male first names for showing respect, while to add the tag 'ben' for female first names for the same purpose. This is similar to putting Mr., Mrs., Ms. etc. in front of the names in the U.S.
5. My father's first name was used as my middle name in most of my documents. The tag ‘sinh' or 'bhai' was interchangeably added in my father's first name.
6. Therefore, the following names refer to the one and the same person:
Surname ABC
Surname ABC XYZbhai
Surname ABC XYZsinh
Surname ABC XYZkumar
The above are the variations of the same name and should not be considered name changes.
So I do hereby solemnly affirm that foregoing are true and correct statements.
Place: Singapore ---------------------
Date: June 19, 2014 DEPONENT

Thanks in advance.
YES...This affidavit is okay...finish it up!
Cheers.

Do we have to have 67 points when we file for application ? I have 65 points and I filed for FSW.

My spouse is US born citizen and I have written a letter saying that we didn't give the IELTS because spouse is born citizen. If my spouse gives IELTS, we will have 70 points. But we sent the FSW file on 14th May and they received it on 20th May. We also mention that if we will have to give it, we will give it as soon as they will tell us.

Will it affect the file ? Will they return my file ?

I am confused now. what do you say?
If your self calculated points are actually 65,your application will be returned to you un-processed regardless of the fact that your spouse is a US citizen.had it been that your spouse is Canadian citizen living in Canada with verifiable evidence, you will be considered .
I will suggest that your spouse write the IELTS while you request for another copy of yours(original) and original ECA copy to send a fresh and new application package.
Cheers

Just reconfirming my IELTS Scores (came today):

L7.5 + S7.5 + R7.5 + W6.5

5 + 6 + 6 + 5 = 22

Correct ?

Don't hope so.....KNOW so.
Dont be surprised to see it on or before Monday....possibly today.
Cheers.

Ok. I will tell him to give ASAP. But will they atleast accept our application?

What I want is not to return the file just for this reason. I will get the date for my husband fir IELTS and will tell him to give ASAP.

but will they accept my app? because if they will return it and then if i will send it back, the cap will be reached. that's the problem now.

@ ami,

Sorry to say, CIC doesn't work on commitments/ comprises etc under any circumstances.

If you want to claim 5 points for Adaptability (Spouse) you must provide his IELTS.. the sooner the better :)

Good luck !
