I would advise you raise a CSE immediately. The auto generated medical form does not address your case and as such important information specific to your case file might have been lost in the process. You never can tell,there might be other information necessary for the furtherance of your application other than just medical examination.
In the meantime, you can proceed with your medical examination to save time while you await their response.

I just got my international passport renewed. Now with new passport number different from what I wrote on the applications.
Do I have to inform AVO about this ?
Or should I just "bone" ?
The old is one is set to expire in May and i feel its less than six months already.
