Someone at AVO has been consistently making this mistake. The form they're actually referring to is Schedule A: Background/Declaration IMM 5669E. IMM 0008 is the generic application form.

Check on the CIC site again. They're human after all.

They made the same mistake when they asked me to send the updated form.
I sent a mail across and their response was vague but indicated they meant IMM5669E
