Madam passport would expire in 2019 while mine in 2017.
I made change of name for her and a paper publication exists for it.
How do I fill the section number 2 of the generic form for personal details of dependant?

I made change of name for her and a paper publication exists for it.
How do I fill the section number 2 of the generic form for personal details of dependant?

Okk for the change of name, which name is in her intl passport? Dts what u shd maintain for consistency afterall her previous names n present names are still valid as per the punblication. In the dependent section, which comes after your section. Once u cllick on add dependent, u fill out the spaces in the box, u will c the space for UCI- input the UCI number for madam and baby, height, eye color etc , etc. U will also see the previous name and current name , pls be careful n maintain what is in the international passport u wish to send as her current name.

I know dt intl passports with maiden name can only be changed in abuja n I dont know how long dt takes bearing in mind d 15 day notification ( though u can ask for extension) but if her intl passport is already in ur name, then know dt no abuja runs for u. Whtever is d case, pls maintain what is in the intl passport ure sending to accra.
