Just fill the form with all your details from age 18 till date and include evidence that you left your job (e.g copy of the acknowledgement of your resignation letter). Thanks a lot for the response...I get your point. We applied in October so I guess that means we have sometime, if I choose to change my name on passport.

If you are not under some sort of pressure to do a name change, then just complete your processing with AVO the way you started it. Reduce the amount of questions that could be asked on your case file. My opinion right there. All the best.
