FSW 2013 Form filling help needed

HI,

I am applying this year under FSW and while i am filling the forms i am facing some problem. Please help me.

Problem:

1) My wife applied CANADA last year as a student and refused, now while she is filling the form SCHEDULE A BACKGROUND DECLARATION which option should be selected for question no.6 from the below?

d) been refused visit or temporary resident visa for Canada or any other country?
e) been refused admission to or ordered to leave Canada or any other country?

2) Is it necessary to provide the same info in my SCHEDULE A BACKGROUND DECLARATION form?

3 should I mention her UCI number in my generic form?

N.B: I am the principle applicant

so i am gonna tick
1. D in my spouse's BACKGROUND DECLARATION form
2. D in my BACKGROUND DECLARATION form and provide detail about her rejection
3. Will Leave blank (UCI) in Generic form
