Some useful info.

Here's the exact quote from the passport specs :

"Passport Specifications

If any of the passports submitted do not meet the following specifications, all passports will be returned to you without visas. This delay will shorten the amount of time that you will have to travel to Canada to become a permanent resident.
1. All names and dates of birth must be printed in each passport in English or French.
2. The given name (first name) of the passport-holder must be clearly printed on the Given Name line of the biodata page of each passport. The surname (last name) of the passport-holder must be clearly printed on the Surname line of the biodata page of each passport. They should match your Updated Information Chart.
3. Your spouse must have their own passport. Your accompanying children should also each have their own passports.
4. Each passport must have at least two empty pages.
5. For your visas to have maximum validity, all passports should be valid for at least 12 months from the date of your medical examinations.
6. All passports must be intact and not worn, torn or otherwise damaged; the laminate on the biodata page must be securely attached. Each page must be securely affixed to the passport; there cannot be any loose pages. Ordinary passports are the only type of passport into which a permanent resident visa may be inserted. Diplomatic, official, special or other similar types of passports are NOT acceptable."

Now that i have those letters in hand, which account for 6 years of my work experience, i feel my consultant was negligent and so i blew off my chance...

No its not fine at all. There is nothing like exp. date for pcc and the pcc must not be more than 3months old. So u must submit new pcc if it is more than 3 months old ok

If sending revised form will give you relaxation, you can send a revised form under a cover letter. Just for your satisfaction, it was exactly same in my case too but I got PER so based on my experience, it shouldn't be a reason for whole package to be returned unless you have made any other critical mistake.

The confusion comes from the word "Applicant". There should have been option to choose/write "Principal Applicant" for ourselves when we are filling the forms and to choose/write "Applicant" for the spouses when our spouses are filling the form. I think, this can be understood by VOs too. For this reason, I believe, it is just considered a typo mistake and accepted for processing. Logically, the form is complete, the only minor mistake is that the places for writing the names & info for Principal Applicant and Spouse are interchanged.

Dear, have you received your application package back or it is still with CIC. To be honest, very few of us were able to get the experience letters in the way CIC wants. All employers do resist in one or other way. I was able to get the experience letters and I attached JDs with them, that worked for me at least.
