She used an outdated form, but she got her file back, updated the application package and sent it again. Good Luck Marta

In instruction guide I found all papers have to put in a 23 cm x 30.5 cm (9″ x 12″) envelope. Can I fold the documents in there? Including IELTS original copy?

2174 changed to 10% for the sake of forums applicants. Acceptance ratio changed to 90% as these are computer professional, rare chance mistake!
2147 changed to 8%. Acceptance Ratio changed to 85%.
1112 Changed to 5% from 3% with same acceptance ratio of 75%.

Rest are around 3% & 75%.

And the rest of us are so dumb that must make mistake....
