Hi mate! this is how i understand, the mistake you did is that in your spouse form you wrote "your name as applicant" and the rest of the form is filled your spouse.

I believe during cross check they will find this mistake by comparing names & signature with other forms. In my view this should not create confusion for them expect to realise that you have put wrong name. How fatal it is no one can say about it. As i dont see that it is creating confusion, in my view it will be OK. So just relax and keep faith, as you cant do anything. I also believe that many people get confused while filling this form, and CIC should be knowing this.

Many thanks for your response Stranger Friendly. It wasn't clearly stated 'Principal Applicant' as in other forms so I was assuming 'Applicant' to mean same. I pray it doesn't harm as I'll be keeping the faith on success.

Seniors, I posted this request earlier, someone should kindly guide me:
My spouse was born in India and she does not have a birth certificate. She only has a record of catholic baptism which indicated her date of birth and parents' names. This is what she has always used in place of birth certificate. Also, she left India as a young child, she did not complete any education there and she does not have a 'marksheet' as stated earlier in this forum.

I have sent our application using this baptismal document.
1. Can our application be returned because of this?
My wife recently told me that during one of her travels, the embassy in our home country, requested for a confirmation of birth. She showed me a handwritten document from the hospital, stating parents' names and her date of birth without indicating her name. The document also has both Tamil and English inscriptions below it.
2. Should I send this document to CIC?
3. If yes to Q2 above, do I need to translate the Tamil aspect, though it looked as if the translation has been done in English (beside the Tamil words). My wife can only speak Tamil, she cannot read it.
4. If I am to send this document, can CIC link it to my file? I do not have a UCI. I am the principal applicant, but my wife had UCI from a previous unsuccessful FSW application.
5. What is the best means of sending the document - email, fax, courier or combinations?

I will appreciate as many inputs as possible from people on this forum.

Thank you.

Before issuance of PER CIC just checks the completeness of the documents. VO then check authenticity of the documents before issuance of the visa.
Inconsistency of documents should be the main reason.

Hi Guys, are there many cases when applicants received PER and then rejected by their respective visa offices? Do you know what the main reasons are?

Rawdah,

In the Additional Family Information form IMM5406E for my spouse I wrote my names where we have 'Applicant' and my wife's names where we have 'Spouse or common-law partner' but the rest of the details including signature is that of my spouse. Can this be a problem leading to rejection? Did anyone get PER filling the form as this way?

Anticipating your best answers and advice.

Thanks!
Rightly said by Stranger Friendly many people get confused while filling this form.
Cic can easily make out this mistake by comparing your details with other forms.
Hope that cic overlook this mistake.

No one can really say how CIC will look into it but I don't think it will be much of an issue. Actually, I initially did the same mistake till I read someone on this forum commenting on this issue and I managed to rectify it before sending the application to CIC.

Two seperate form should be filled by the principal applicant and his spouse for Additional family information
1) Principal Applicant is the Applicant giving all his details with his signature
2) Spouse is the Applicant giving all her details with her signature

REMEMBER THE FORM SAYS APPLICANT AND NOT PRINCIPAL APPLICANT

Ok so now I know I made a big mistake...I filled 2 additional family information forms 1 for me n my family details n 1 for my spouse n her family details... my spouse signed her form but the mistake we did was on her form I wrote my name as applicant n her name as spouse... so I assume my application should be home soon..

please help what should i do now?
Doesn't matter. I have sent the same, and did not face any issue. They are national ID's which I am sure no need for any translation.
