You could have added a plain sheet with the form explaining the addendum. Filling forms is a different process from the attachments. In the column of NAME, if you write "Refer attached Passport", it would not make any sense. Its the same with job duties in Schedule 3. They have kept a provision of attaching extra pages if you need to give more info. So if you cannot write it in the form, you should attach extra sheet mentioning the job duties in plain paper and to support that information, you provide a reference letter. That additional sheet should be attached to the form itself, so the VO will not have a hard time looking for it. Now in your case i still feel that it might get through, as they might take it that the additional sheet is misplaced, but your reference letter is available. So pray for a lenient Visa Officer. Bcoz sending attachments at this stage might create confusion. But regarding sending additional sheets at this stage, other senior members will be in a good position to reply.
