Forms for Online Payment for Extension

I'm (American) applying to extend my visitor visa. My husband (Canadian) paid the $75 fee online, and we printed the receipt. However, the forms that they ask us to fill out are confusing us.

Under the IMM 5401 "Receipt", there's these fields:

Payer Information:
* Name
* Client Date of Birth
* Client ID
* Address
* Postal Code
* Telephone Number

Are they asking for the actual payer (my husband) or actually my (the applicant) information?

We were going to fill out his information, but then we didn't know what to do about the fields for "Client" specific information. And when I went to fill in my information, my American Postal Code is too short to fill out the 6 blank boxes.

Whose information does it want?
