Form IMM0006 for extended family members -- problems downloading?

I tried opening up this form this morning and am only seeing one page where it asks you to check whether you're a citizen or permanent resident. How many pages does this form actually have and why am I only seeing one page? Very frustrated.

I would like to fill out this form for my brother to come here to canada together with my parents, but who will sign for him if he is a minor.

3pages. Try to put check on the box then it will pop out.
 
can your extended fam member abroad sign and email you imm0006 so you can then proceed to the notary for signatures ?

OR

do they have to regular mail you with original signatures ?
 
Has anyone from India got the visitor visa approved under the Extended family member program recently?
