write all names in english as well as in your native language.

so we should write in both language in same form?

Yes type it in English then print the form and write the name in the same box in your native language, if there is not enough space then attach a separate sheet.

Hi What u mean by separate sheet..is that the same form given by them or a blank sheet...i cant write my full name in sinhala language..i need some additional space..
