Forgot to mention previous visa refusal :( Please help

Hey Everyone,
Urgent Query for my friend who has applied for PR under CIC.
He got an email from IRCC requesting a "US student visa refusal letter" which was back in 2016 and has now lost the letter.
So does anyone know how to get that letter?
Or what steps does he need to take now?
Kindly please help with this if anyone knows anything.
The letter from the IRCC officer says:
Please provide USA student visa refusal letter This must be received at this office by 2021/08/18

The letter from the IRCC officer says:

You have to send and request for Freedom of Information Act in Department of state if im not mistaken.
If you cannot get copy of refusal letter, just inform with like you apply from the state with proof. So you may get more time. I am not sure but still @legalfalcon can help you out more. Thanks
He did send an email to US Consulate mumbai and got reply that they don't keep any record of such document.
Hi @legalfalcon , would appreciate if you can give us some advice - My wife has already been granted a study permit and is already in Canada now. Today, we realized that she answered one of the forms incorrectly and forgot to mention her previous US tourist visa refusal in 2010 since it was a long time ago already. We checked the forms that were submitted to IRCC and found out that she answered "NO" to the question asking whether she has been refused a visa or permit, denied entry or ordered to leave Canada or any other country or territory. In this case, should she inform IRCC of this error or should she just declare this mistake when she applies for PGWP? We're worried that this oversight will cost her study permit and her chances of getting a PGWP and PR. Would really appreciate it if you can help us on this matter. thank you!

After the visa is issued, the application is closed. So informing now will not help. Instead, when you apply next, make sure you address this in an LoE.
To be on the safer side you can send a webform and inform now and keep it as a record.

And told about study visa refusal and visitor extension refusal of canada
But
I forgot to told my canada work permit refusal in 2016
What should i do now?
