I want to practise medicine in Canada, apart from the licensing exams, do I need ielts too? If I do, which kind of ielts, and do I take the ielts before or after the exams(mccqe)
For express entry you need to have your degree assessed, since it's a regulated profession you would ussualy have to do an ECA and an additional one from the Medical Council of Canada.
That doesn't allow you to practice, you'd then need to be licenced by the province in which you'd be practicing. It also depends on whether you have an specialty and how long have you practiced medicine for.

The biggest issue that prevents most IMGs from practicing medicine in Canada is that most (not US, South African, AUS, NZ and some European grads) IMGs need to repeat their residencies. The issue is that there are very few residencies after Canadian students have selected their own residencies. On top of that many specialists can’t find jobs when they graduate. If you want to continue practicing medicine Canada is not the ideal place to immigrate. Other countries provide a much better chance of being able to practice.
