Whenever applicable, please bring the following:
Old chest x-ray films not less than 3 months prior to the medical examination
Detailed medical certificate regarding a previously-treated or currently managed clinically significant illness which should contain the following information:
duration and date of treatment
surgical procedure or medical intervention done
complete final diagnosis
outcome of treatment or treatment plans and prognosis, if available

For the convenience of those who come early, the guards may allow applicants to enter St. Luke's Medical Center Extension Clinic premises as early as 4:30 AM but the schedule of operation of each working area will be observed.

REMINDERS:
Applicants are advised to take their meals prior to coming over for their medical examination since food is not allowed within the premises and all the initial blood and urine tests do not require a fasting state. There are restaurants near St. Luke's Medical Center Extension Clinic and drinking fountains are available within the building.
Females within the reproductive age group are advised not to schedule their medical examination during their menstrual period. Otherwise, they will be required to come back one week after the last day of their menstruation.
Note:
Medical results and documents will be released not on a first-come first-served basis but depending on the physical examination, x-ray and laboratory results, which will be conducted and processed in accordance with the guidelines set by the Canadian Embassy - Health Management Branch.
The medical report, and chest x-ray are directly submitted by St. Luke's Medical Center Extension Clinic to the Canadian Embassy in Makati. For offshore applications, medical reports and x-ray films are submitted via courier.
